This Cases on the Web activity introduces 2 cases and presents 5 clinical decision points related to the care of HIV-infected patients with human papillomavirus-related anal dysplasia or anal squamous cell carcinoma. At each clinical decision point, learners should select the option that they believe is most sound and read the presenters’ explanation of that option. Selecting the correct option links the learner to an extended discussion of related medical findings, research-based evidence, and case management considerations.

Activity Objectives

On completion of this activity, the learner will be able to:

  • Discuss the epidemiology of human papillomavirus infection of the anus and the incidence of anal dysplasia and anal squamous cell carcinoma among HIV-infected persons.

  • Identify the risk of anal squamous cell carcinoma in HIV-infected persons with anal dysplasia that is detected on cytologic screening.

  • Explain the use and limitations of digital rectal examination and anal Papanicolaou testing for screening of anal dysplasia and anal carcinoma.

  • Compare the available treatment options for anal dysplasia in HIV-infected persons.

  • Discuss the use of condoms, human papillomavirus vaccine, and antiretroviral therapy to prevent anal dysplasia and anal squamous cell carcinoma.

Assessment of Needs

The International AIDS Society–USA offers this state-of-the-art activity as part of a nationwide CME effort for HIV physicians on the evolving challenges of HIV disease. This activity examines issues relevant to the care of HIV-infected patients with human papillomavirus-related anal dysplasia or anal squamous cell carcinoma.

Intended Audience

This Cases on the Web presentation is for physicians at all levels of HIV practice, both advanced and basic. Advanced-level HIV physicians are those who:

  • Have a solid, working knowledge of HIV disease management.

  • Provide comprehensive or specialty care for at least 10 patients with HIV and AIDS or are involved in HIV and AIDS clinical trials or investigations.

  • Have completed at least 10 hours of CME in HIV and AIDS medicine in the past 2 years.

Basic-level HIV physicians are those who are:

  • New to the practice of HIV medicine or who intend to start providing care for patients with HIV and AIDS.

  • Seeking to refresh their understanding of the standard elements of HIV care.

This activity is also relevant to nurse practitioners, physician assistants, nurses, and other health professionals who provide care for people with HIV disease.

CME Accreditation

The International AIDS Society–USA is accredited by the Accreditation Council for Continuing Medical Education to sponsor continuing medical education for physicians.

The International AIDS Society–USA designates this educational activity for a maximum of 1.0 AMA PRA Category 1 Credit™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

This CME activity is offered from December 1, 2009 to December 1, 2010. Participants who successfully complete the activity posttest and submit the evaluation and registration forms are eligible to receive CME credit. Physicians (MDs, DOs, and international equivalents) may receive CME credit for completing this activity. Nonphysician health care practitioners will receive a certificate of attendance.

John Koeppe, MD
Dr Koeppe is an assistant professor in the Division of Infectious Diseases of the Department of Medicine at the University of Colorado Denver School of Medicine in Aurora. He provides care for HIV-infected persons in the Infectious Disease Group Practice at the University of Colorado Hospital, Aurora. Dr Koeppe’s research interests are directed at the primary care of persons with HIV infection and the management of HIV comorbidities.

Steven C. Johnson, MD
Dr Johnson is a professor in the Division of Infectious Diseases of the Department of Medicine at the University of Colorado Denver School of Medicine in Aurora. He is director of the Infectious Disease Group Practice at the University of Colorado Hospital, where his practice is directed at the care of persons with HIV infection. Dr Johnson’s research interests include the improvement of HIV outcomes and clinical trials of antiretroviral therapy.

Amy M. Sitapati, MD
Dr Sitapati is an associate clinical professor at the University of California, San Diego School of Medicine. She serves as associate director of the Owen Clinic at the University of California, San Diego Medical Center, a primary care Ryan White funded academic HIV practice. Dr Sitapati has been actively performing anal colposcopy for more than 5 years and serves as a regional expert in the field of anal dysplasia.
