This Cases on the Web activity consists of 4 cases and 7 clinical decision points, each focusing on providing care for an HIV-infected patient experiencing possible immune reconstitution inflammatory syndrome. At each clinical decision point, learners should select the option that they believe is most sound and read the presenter’s explanation of that option. Choosing the best option links the learner to an extended discussion of related medical findings, research-based evidence, and case management considerations.

After reviewing the cases, learners may link to the continuing medical education (CME) posttest questions and the course evaluation, both of which must be completed and submitted to receive credit. Please see the post test form and course evaluation form for further instructions.
On completing this activity, the learner will be able to:
- Define clinical criteria for the diagnosis of the immune reconstitution inflammatory syndrome.
- Discuss considerations in starting antiretroviral therapy in patients with opportunistic infections.
- Describe the approach to management of patients with immune reconstitution inflammatory syndrome.
The International AIDS Society–USA offers this state-of-the-art activity as part of a nationwide CME effort for HIV physicians on the evolving challenges of HIV disease. This activity examines issues relevant to managing HIV-infected patients with immune reconstitution inflammatory syndrome (IRIS).
This online CME activity is designed for physicians who are actively involved in HIV and AIDS care. Specifically, these activities have been designed for physicians who:
- Have a solid, working knowledge of HIV disease management.
- Provide comprehensive or specialty care for at least 10 patients with HIV and AIDS or are involved in HIV and AIDS clinical trials or investigations.
- Have completed at least 10 hours of CME in the area of HIV and AIDS medicine in the past 2 years.
This activity is also relevant to nurse practitioners, physician assistants, nurses, and other health professionals who provide care for people with HIV disease.
The International AIDS Society–USA is accredited by the Accreditation Council for Continuing Medical Education to sponsor continuing medical education for physicians.
The International AIDS Society–USA designates this educational activity for a maximum of 2.0 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

This CME activity is offered from December 23, 2009 to December 23, 2010. Participants who successfully complete the activity posttest and submit the evaluation and registration forms are eligible to receive CME credit. Physicians (MDs, DOs, and international equivalents) may receive CME credit for completing this activity. Nonphysician health care practitioners will receive a certificate of attendance.

Jaime C. Robertson, MD
Dr Robertson is assistant professor in the Department of Internal Medicine, Division of Infectious Diseases, at the University of Cincinnati College of Medicine in Ohio. He has conducted numerous HIV clinical trials, and his research interests are in the immune reconstitution inflammatory syndrome and the prevention and treatment of human papilloma virus-related anal carcinoma.
Carl J. Fichtenbaum, MD
Dr Fichtenbaum is professor of clinical medicine and associate program director for the Internal Medicine Residence Program in the Department of Internal Medicine, Division of Infectious Diseases, at the University of Cincinnati College of Medicine. He is the principal investigator of the National Institute of Allergy and Infectious Diseases (NIAID)-funded AIDS Clinical Trials Unit at the University of Cincinnati. His research interests include HIV-related dyslipidemia, cardiovascular disease, and the immune reconstitution inflammatory syndrome.

William J. Burman, MD
Dr Burman is medical director of the Infectious Disease Clinic at Denver Public Health, a system providing comprehensive care to approximately 1000 patients with HIV infection, and a staff member of the Denver Metro Tuberculosis Clinic. Dr Burman’s research focuses on the design and implementation of randomized clinical trials to improve the treatment of tuberculosis (through the Tuberculosis Trials Consortium) and HIV infection (through the International Network for Strategic Initiatives in Global HIV Trials).
